Instructions for filling of online application form

  1. Application form must be applied online only which can be downloaded from council website www.mpmissionexcellence.org or www.mpcost.nic.in

  2. The eligible candidates must apply online and after submission take a printout which should be completed and duly signed by self, parent/guardian, affix a photograph and get attested and forward by Principal of school (this is very important and necessary). Candidate must keep a photocopy of completed application form before sending to council.

  3. The application form must be accompanied by attested photocopies of certificates whichever applicable and attested mark sheet of previous year (year 2014-15).

  4. The applicants must fill the column like Name, Fathers Name, Total Marks obtained out of grand total in the online application form in English only. Those schools where results are given in grade must convert the grade into total marks obtained out of grand total and specify in the mark sheets.

  5. Students currently studying in the class in year (2015-16), Below Poverty Line (BPL) candidates, physically challenge candidates and respective board should select the appropriate option in the application form.

  6. Caste should be mentioned at appropriate place and supporting caste certificate must be attached with the application form otherwise the candidate will be treated in general category; General category students may not send any caste certificates.

  7. Students of category (belonging to physically challenged category should have 40% or more disability) must produce certificate of the same.

  8. For correspondence full residential address along with pin code and also the address of school along with pin code should be filled in the appropriate place and mobile number.

  9. Students studying in present session (2015-16) in a particular district will be considered from the same district as the selection will be based on region basis.

  10. Please mention your telephone number along with STD code and Mobile number

  11. The application form available on council’s website mpcost.nic.in or mpmissionexcellence.org must be applied, downloaded and sent along with supportive documents, any other application form will not be considered.

  12. The selection of the candidates will be solely based on information’s provided by them in the application along with requisite documents hence any false information or incorrect information will be the sole responsibility of the candidates and the candidates will be disqualified at any stage.

  13. The applications form must reach the council on or before the stipulated date. Forms received after stipulated date will not be considered.
