1. Students must fill application form online using website www.mpmissionexcellence.org or via link available in www.mpcost.gov.in The online form must be filled in English only.
2. The Scheme is only for students studying in class 8th & 9th in any school of Madhya Pradesh are eligible to apply.
2.1. The Students with minimum 75% and above in the previous examination i.e. 2024-25 are eligible to apply.
2.2. Application along with marksheet of previous year (2024-25), duly verified by the Principal must be uploaded.
3. All the required documents attested by the Principal not older than 5-10 days from the date of filling online application form will only be considered.
1. In many cases mark sheet show annual marks & Grand Total, in such cases it is mandatory to fill Grand Total marks mentioned in the mark sheet. If the students fill the annual marks it will be not considered for selection, hence application will be rejected.
2. School where marks are given in Grades, in this case before uploading the mark sheet, students must submit grade conversion with percentage duly verified by the school’s Principle. Specify total marks obtained out of grand total specified in the mark sheet in case of grades. The proforma of Grade Conversion can be downloaded from website - www.mpmissionexcellence.org
1. The marksheet of previous year i.e. 2024-25 duly verified by the Principal must be uploaded in the relevant field assigned on online application form.
2. The clause (A)2.2 is mandatory to all students. In absence of this, the form will be rejected.
3. Students belonging to Below Poverty Line (BPL) and Specially Abled Person in current year (2025-26) will select appropriate option in the drop down menu. The desired certificates must be verified by the principal (Seal & Sign) in proper format. Unverified certificates will not be entertained & application will be rejected.
Note: Specially Abled Person with 40% or more disability will be considered under this category.
4. Certificate of Caste for students belonging to SC/ST/OBC category must submit certificate duly verified by the Principle before uploading the document during online application filling. Unverified certificates will not be entertained and liable to be rejected. Caste to be selected using scroll down menu. Students belonging to reserve category must upload the document as per instructions otherwise candidate will be taken in GENERAL category.
5. Address for correspondence (Full Postal Address) with pin-code, postal address of school with pin code, DIAS number of school, Telephone number with STD code/mobile number should be filled in the appropriate box in online form.
6. Aadhar Card Number & copy of Aadhar card to be uploaded.
7. Students studying in present session (2025-26) in a particular district will be considered from the same district.
1. Candidate must keep a printout of completed application form along with all the documents. This form will be required by MPCST.
2. The selection of the candidates will be solely based on information provided by them in the application along with requisite documents hence any false information or incorrect information will be sole responsibility of the candidate and the concern school and candidates will be disqualified at any stage.
3. Application form accompanied by attested photocopies of certificates whichever is applicable in proper format & verified by the Principal of School will only be considered.
4. Students are requested to fill availability of Android/ IOS/ Window/ Mobile at the appropriate box in the online application form.
5. Students may also download the PDF from the Website for detail required for online yatra.
6. The Candidates must apply online and after submission take a printout, which should be completed and duly signed by self, parents/guardian, affix a photograph and get attested and forwarded by Principal of School.
1. अभ्यर्थी को परिषद की वेबसाईट www.mpmissionexcellence.org अथवा www.mpcost.gov.in पर उपलब्ध लिंक पर आवेदन पत्र को ऑन-लाइन भर सकते है। ऑन-लाइन फार्म को अग्रेज में भरना अनिवार्य है।
2. यह योजना विज्ञान के कक्षा 8वीं एवं 9वीं के विद्यार्थी जो किसी भी मध्य प्रदेष के स्कूल में अध्ययानरत है वे विज्ञान मंथन के लिए पात्र है।
2.1 वर्ष 2024-2025 सत्र की परीक्षा में न्यूनतम 75 प्रतिषत एवं उससे अधिक प्राप्तांक वाले विद्यार्थी ही आवेदन हेतु पात्र होंगे।
2.2 आवेदन के साथ अनिवार्यतः (2024-2025) सत्र में उत्तीर्ण परीक्षा की अंकसूची, प्राचार्य द्वारा सत्यापित एवं अग्रेषित को अपलोड करना अनिवार्य है।
3. सभी प्रमाण पत्र जो प्राचार्य द्वारा सत्यापित एवं अग्रेषित है वे फार्म भरने के दिनांक से 5 से 10 दिन से ज़्यादा पुराने नही होने चाहिए।
1. विद्यालय जहां अंकसूची में महायोग दिया जाता है उस अवस्था में आवेदन में महायोग भरना आवष्यक होगा । अगर विद्यार्थी वार्षिक परिणाम भरता है तो उसे चयन के लिए नही देखा जायेगा । अतः आवेदन निरस्त माना जावेगा।
2. जहां परीक्षाफल ग्रेड में प्रदाय किये जाते है वहाॅ मार्कशीट अपलोड करने के पहले विद्यार्थीे ग्रेड को प्रतिषत मे परिवर्तित कर विद्यालय के प्राचार्य से सत्यापित करे । जिस अंक सूची में ग्रेड दिये गये है वहा प्राप्तांक/पूर्णांक भरे। ग्रेड परिवर्तन का प्रारूप वेबसाइट से डाउनलोड करे।
1. आवेदन के साथ अनिवार्यतः ( 2024-2025 ) सत्र में उत्तीर्ण परीक्षा की अंकसूची विद्यालय के प्राचार्य द्वारा सत्यापित एवं अग्रेषित कर आवेदन वेबसाइट पर अपलोड करना अनिवार्य है।
2. क्रमांक (अ)2.2 सभी कक्षा 8वी से 9वी के विद्यार्थीयो को अनिवार्य है।
3. गरीबी रेखा के नीचे जीवन यापन करने वाले अभ्यर्थी, निःशक्तता आदि के उचित विकल्प चयन करें। सभी प्रमाण पत्र प्राचार्य द्वारा अग्रेषित (मोहर एवं हस्ताक्षर) तथा उचित फॉर्मेट में होने चाहिए।
नोटः- दिव्यांग 40 प्रतिशत या उससे ज्यादा दिव्यांगता का प्रमाण पत्र आवेदन के साथ संलग्न करने के उपरान्त ही इस वर्ग में आवेदन कर सकते है।
4. जाति वर्ग (अनुसूचित जाति जनजति एवं पिछडा वर्ग) के विद्यार्थी उचित विकल्प का चयन करे एवं इस आशय का प्राचार्य द्वारा सत्यापित प्रमाण पत्र आवेदन अपलोड करते समय संलग्न करें। अन्यथा उसे सामान्य श्रेणी में ही माना जायेगा।
5. पत्राचार हेतु निवास का पूरा पता ,पिन कोड सहित एवं अध्ययनरत् विद्यालय का नाम व पता, एवं स्कूल का डाइस नम्बर,मोबाइल नम्बर, पिन कोड़ सहित सही रूप से भरे। निवास का दूरभाष क्रमांक, दूरभाष कोड़ सहित या मोबाइल नम्बर भी भरे।
6. आधार कार्ड नम्बर तथा आधार कार्ड की छायाप्रति भी अपलोड करे।
7. चयन में इस वर्ष आप जिस जिले में अध्ययन कर रहे है, वही क्षेत्र मान्य होगा ।
1. अभ्यर्थियों को ऑनलाइन आवेदन करने के उपरांत आवेदन का प्रिन्ट आउट निकालना होगा, सत्यापित प्रमाण पत्र संलग्न कर अपने पास अवष्य रखे । यह आवेदन मध्य प्रदेश विज्ञान एवं प्रौद्योगिकी परिषद को सत्यापन तथा छात्रव्ति परीक्षा (अगर आयोजित की जाती है तो) के समय प्रस्तुत करना होगा ।
2. अभ्यर्थियों का चयन ऑनलाइन आवेदन भरे हुए पत्रो के आधार पर ही किया जायेगा। अतः जानकारियों के पूर्ण व सही होने का पूर्ण उत्तरदायित्व अभ्यर्थी का ही होगा। कोई भी गलत जानकारी/दस्तावेंज प्रस्तुत करने पर एवं अपूर्ण एवं त्रुटिपूर्ण जानकारी की दशा में आवेदन किसी भी समय पर निरस्त किया जा सकता है। जिसकी जिम्मेदारी स्वयं आवेदक एवं स्कूल की होगी।
3. अम्यर्थियों द्वारा प्रेशित सभी प्रमाण पत्र की प्रतिलिपि वर्तमान में अध्ययनरत् विद्यालय के प्राचार्य द्वारा सत्यापित एवं अग्रेषित हो तथा उचित फारमेट में हो । वे ही आवेदन मान्य किये जायेगे।
4. विद्यार्थीयो से अनुरोध है वे एंड्राइड/आई.ओ.एस/विंडोस, मोवाइल आदि की उपलबधता ऑनलाइन आवेदन के नियत खाने मे दर्षाए ।
5. विद्यार्थी वेब-साइड पर जाकर क्रमांक 5 के दिशा निर्देश को ऑनलाइन यात्रा के लिए डाउनलोड कर सकते है।
6. अभ्यर्थियों को ऑनलाइन आवेदन करने के उपरांत आवेदन का प्रिन्ट आउट निकालना होगा, जिसमें समुचित स्थान पर अपने हस्ताक्षर, पालक के हस्ताक्षर व अपना पासपोर्ट साइज फोटो निर्धारित स्थान पर चिपकाकर सत्यापित प्रमाण पत्र संलग्न कर अपने विद्यालय के प्राचार्य द्वारा सत्यापित और अगे्रषित कर अपने पास अवश्य रखे ।
